Before scheduling an appointment, please review the following criteria to see if you qualify for our comprehensive pain management consultation:
Current Pain Management Status
Are you currently under pain management? If you have been discharged from another pain practice, please let us know the reason. If you have a discharge letter, we may need to review it.
Medication History
What pain medications are you currently taking? Note that we may need to discuss medication rotation or tapering if you’re on high dosages like Oxycodone 30mg.
Additional Medications
Are you taking any anxiety or sleeping medications? If so, we’ll need to coordinate care with your prescribing physician.
Insurance and Payment Information
We accept most major insurances and provide a complimentary benefits check to confirm coverage. If you don’t have insurance, the full consultation cost is $279. Please inform us about your insurance type or if you are using cash.
