1. Current Pain Management Status:
   • Are you currently under pain management?
   • If you have been discharged from another pain practice, please let us know the reason.
   • If you have a discharge letter, we may need to review it.
2. Medication History:
   • What pain medications are you currently taking?
   • If you’re on high dosages, such as Oxycodone 30mg, we may need to discuss medication rotation or tapering.
3. Payment and Insurance Information:Consultation Fee
   • Comprehensive pain management consultation: Insurance coverages vary, discussed upon intake, we verify your insurance to see if you are covered for additional services and tests
   • IV therapy consultation and 1^st drip : $179 for ALL patients, $25 to $150 individually tailored IV bag programs available
4. Additional Medications:
   • Are you taking any anxiety or sleep medications?
   • If so, we’ll need to coordinate care with your prescribing physician.
5. Referral Source:
   • How did you hear about us?
   • Whether through a referral, online search, or another source, we’d love to know how you found us.
